2020-2021 PK3 and PK4 REGISTRATION INFORMATION

EARLY REGISTRATION DATES:  3/30, 3/31, 4/1 and 4/3

Welcome to the Clayton Public School District.

We offer tuition-free, full-day programs for 3-year and 4-year old students.

Your child must be three (3) years of age by October 1, 2020 to enroll in our PK3 program

OR your child must be four (4) years of age by October 1, 2020 to enroll in our PK4 program.

Students will be placed in pre-school on a first come basis.

Once seats are filled, students are placed on a waiting list.  After the Early Registration period is closed, registration appointments will not be available until July 1st.

REGISTRATION PROCESS

  1. Get a registration packet from the Board of Education Office at 350 East Clinton Street in Clayton OR download and print by selecting one of the links below:

  1. Make a registration appointment. Packets are submitted BY APPOINTMENT ONLY.

  1. Bring completed registration packet and following documentation to appointment.
    1. PROOF OF RESIDENCY – must display parent or guardian name
      1. Lease/rental agreement OR mortgage/deed/settlement OR recent property tax bill and
      2. Any utility bill issued within the last 30 days OR mail received from any federal, state or local government agency in the past six months
    2. PROOF OF CHILD’S AGE, IDENTITY, GUARDIANSHIP and CUSTODY: Birth certificate, passport, hospital birth record, adoption record, baptismal certificate, or legal custody papers (if applicable).
    3. HEALTH DOCUMENTS REGARDING STUDENT: Immunization record with dates for each immunization AND physical examination signed by child’s Health Care Provider and dated within one year of registration.  Medical or religious exemptions must be submitted in writing.  If your child has allergies, seizures or asthma – please ask for additional health forms during registration.

** Registration information regarding grades KDG through 12th grade will be available on June 1st **

2019-2020 REGISTRATION INFORMATION

Welcome to the Clayton Public School District.  We are accepting new students who will be entering grades PK3 through 12th in September.  Parents and/or guardians may pick up registration packets at the Board of Education office OR download and print via the links on this page.  Completed registration packets are submitted to the Board of Education office located at 350 East Clinton Street.

 

STEP 1SCHEDULE REGISTRATION APPOINTMENT – appointment required (no walk-ins)

Summer registration period is from July 1st through August 29th.  Only one (1) student per appointment.  Please schedule multiple appointments if registering more than one student.  Click this link to schedule registration appointment(s):   https://claytonps.org/registration-booking/

 

STEP 2: GET A REGISTRATION PACKET

You can pick-up a registration packet from the Board of Education office located at

350 East Clinton Street in Clayton.  Office hours are 8am to 4pm, Monday – Friday.

You can download and print via one of the links below.  Please note packets are approximately 20 pages – DO NOT print double-sided.

 

STEP 3: PREPARING FOR YOUR APPOINTMENT

Completely fill out the registration packet.  There is a notary available at registration for your convenience.  Review the cover sheet of the registration packet AND make sure you bring the required documents.  You may be asked to reschedule your appointment if your registration packet is incomplete and/or missing required documents.
