Instructions for Transcript Requests & Education Verifications

All transcript requests are processed by the Guidance Secretary and are available to students who have graduated from Clayton High School, Clayton, New Jersey. All requests must be in writing. We cannot accept requests over the telephone. Transcript Release Forms are available at the High School and emails from the former student’s current email account are accepted. Send your email to transcripts@claytonps.org with the following information:

  • Student Name (If there has been a name change, be sure to provide name used while attending Clayton Public Schools)
  • Current Address
  • Date of Birth
  • Year of Graduation
  • Telephone Number
  • Please be sure to tell us how you wish to receive your transcript and include:
    • Contact you for local pick-up
    • Provide email address where transcript is to be sent (to you, a college, university, or other)
    • Provide complete mailing address where you want your transcript sent (to you, a college, university, or other)

If you are a college, university, or outside company interested in obtaining a transcript or education verification for a former student, your inquiry must include the above information, together with a release signed by the former student. You can also send your request via fax to 856-863-0808. The usual turnaround time is between 1 – 2 business days from receipt.
